Promoting Women’s Health and Healthy Births Initiative
Purpose: Improve women’s health needs to increase the likelihood of healthy births.

Increase the number of health education and/or health care access opportunities with women of child-bearing age.

Increase the number of women with a previous LBW birth or at increased risk of LBW engaged in case management.

Increase women’s knowledge about healthy behaviors and opportunities.

Increase the number of community partners engaged in referral and coordination activities to support women and their families.

Increase the percentage of women engaged who understand and develop personal reproductive life plans.
Activities:
•Reduce and eliminate financial housing stressors and increase the number of low-income, at-risk, homeless, and pregnant individuals.
•Promote a healthy lifestyle while pregnant through education and nutritional resources.
•Provide access to health screening services, awareness campaigns, and disease measures educational classes through targeted health fairs as preventative.
